A1C and fasting glucose both provide useful information about blood sugar, but they do not measure exactly the same thing. A1C estimates your average glucose exposure over roughly the previous three months, while fasting glucose measures your blood glucose at one point in time after an overnight fast. Because the tests look at different parts of the picture—and because each has limitations—their results can sometimes differ.
One laboratory value should not be interpreted in isolation. Your symptoms, health history, medications, recent illness, pregnancy status and other laboratory findings may all affect what a result means. At the same time, a result in the normal range does not automatically suggest that a hidden blood-sugar disorder is present, and symptoms such as fatigue, cravings or weight changes are not enough to diagnose one.
This guide explains the recognized diagnostic ranges used for adults who are not pregnant, why A1C and fasting glucose may not agree, and when a result deserves follow-up with a qualified medical professional.
What Blood Glucose and Insulin Do
Glucose is a major source of energy for the body’s cells. After you eat, carbohydrates are broken down into glucose and released into the bloodstream. The pancreas produces insulin, a hormone that helps glucose move from the blood into cells where it can be used or stored.
When the body does not make enough insulin or does not respond to it effectively, glucose can remain elevated in the blood. Persistently elevated glucose can occur with several conditions, including prediabetes and diabetes, but diagnosis requires appropriate clinical testing. Type 1 diabetes is an autoimmune disease in which the body destroys insulin-producing cells. Type 2 diabetes develops through a complex combination of genetic, metabolic, environmental and behavioral factors; it should not be reduced to a simple matter of willpower or described only as a “lifestyle disease.”
A1C and Fasting Glucose Measure Different Things
A1C
The A1C test measures the percentage of hemoglobin in red blood cells with glucose attached to it. It provides an estimate of average blood glucose over approximately the previous three months, with greater influence from the more recent weeks. Fasting is not required.
Fasting plasma glucose
A fasting plasma glucose test measures glucose at a single point in time, generally in the morning after at least eight hours without food or caloric drinks. It can be affected by short-term factors such as illness, stress, sleep, medications, recent activity and whether the fasting instructions were followed.
Recognized Diagnostic Ranges
The ranges below are standard diagnostic criteria for nonpregnant adults. They are not personalized treatment targets. Pregnancy uses different testing and cutoffs, and home glucose-meter readings are not used by themselves to establish a diagnosis.

An abnormal screening result is not a diagnosis to make on your own. When clear symptoms are not present, health professionals generally confirm a diabetes-range result with a second measurement, either by repeating the same test or using another accepted test. If two different tests disagree, the test above the diagnostic threshold is usually repeated.
Why A1C and Fasting Glucose Can Disagree
A normal fasting glucose with an A1C in the prediabetes range—or the reverse—is possible. The tests measure different time periods and do not always identify the same people. A fasting result is a snapshot and has meaningful day-to-day biological variation. A1C reflects a longer period, but it depends on red blood cells and hemoglobin behaving as expected.
A1C can be less reliable in some circumstances, including certain anemias, recent blood loss or transfusion, some hemoglobin variants, kidney or liver disease and pregnancy. Medications and conditions that alter red blood-cell turnover may also affect it. When A1C does not fit the fasting glucose result or the broader clinical picture, a clinician may repeat testing or choose a different validated test rather than assuming either number tells the entire story.
Context matters, but context is not a reason to dismiss recognized diagnostic thresholds or assume that every normal test conceals disease. It is a reason to ask whether the right test was performed under the right conditions and whether follow-up is clinically appropriate.
Symptoms Cannot Diagnose a Blood-Sugar Problem
Fatigue, food cravings, difficulty concentrating, mood changes and weight changes can occur for many reasons. They may justify a broader health discussion, but they do not establish insulin resistance, prediabetes or diabetes. More typical symptoms of significant hyperglycemia can include increased thirst, frequent urination, blurry vision and unexplained weight loss. Seek prompt medical evaluation for these symptoms, especially when they are new or worsening. Severe dehydration, persistent vomiting, abdominal pain, rapid or labored breathing, confusion, fainting or a very high glucose reading can signal an emergency and should not wait for a wellness consultation or routine laboratory review. Call 911 or seek emergency care.
What May Happen After an Abnormal or Conflicting Result
The appropriate next step depends on the result and your circumstances. A clinician may confirm the result, review medications and conditions that can affect glucose, or use another accepted test such as an oral glucose tolerance test. Testing that establishes diabetes does not by itself determine whether a person has type 1, type 2 or a less common form; additional clinical evaluation may be needed.
If your result is in the prediabetes range, it is an opportunity for follow-up—not a personal failure and not a guarantee that diabetes will develop. Evidence-based care may include individualized nutrition, regular physical activity, sleep and weight-management support when appropriate, medication in selected situations, and repeat testing at an interval chosen with a healthcare professional.
Practical Steps That Support Metabolic Health
- Choose a sustainable eating pattern. Emphasize vegetables, fiber-rich foods, minimally processed carbohydrates, adequate protein and unsaturated fats in a way that fits your health needs and culture.
- Limit sugar-sweetened beverages. Water and other unsweetened drinks can help reduce added-sugar intake, but water should not be described as a way to “flush” excess glucose from the body.
- Move regularly. Walking, resistance training, aerobic exercise and higher-intensity activity can all be useful when appropriate. The best plan is one that is safe, individualized and sustainable; high-intensity interval training is not universally required.
- Prioritize sleep and follow-up care. Sleep, stress, medications and medical conditions can influence glucose regulation and deserve consideration.
Dietary supplements are not substitutes for diagnostic evaluation, prescribed treatment or established nutrition and activity strategies. Evidence for products marketed to lower blood sugar is inconsistent, and some supplements can interact with diabetes medications or increase the risk of hypoglycemia.
